Affordable Care Is Not Out of Reach.
We accept most major insurance plans, including Medicaid. Sliding scale fee discounts are available for eligible uninsured patients based on household income and family size, so that cost is never a reason you can't get the help you need.
Accepted Insurance
We are in-network with a wide range of insurance providers, including Medicaid, Medicare, and major commercial plans. If you're not sure whether we accept your insurance, call us and we'll check for you.
Replace with actual insurer logos before launch. Call us to verify your specific plan and benefits.
Our team can also help you understand what your plan covers, including deductibles, copays, and any prior authorization requirements. Call (812) 580-9285 and we'll look it up for you.
Sliding-Fee Discount Program
We believe that quality mental health and recovery care should be available to everyone — regardless of income. Sliding scale fee discounts are available for eligible uninsured patients based on household income and family size.
If you are uninsured or underinsured, you may qualify for significantly reduced fees. To apply, you'll be asked to provide basic documentation of your household income. Our team will walk you through the process — there's nothing to be embarrassed about, and we won't ask for anything we don't need.
Self-Pay Rates
If you choose to pay out of pocket or don't have insurance, we offer straightforward self-pay rates.

|---|---|
| Initial psychiatric evaluation | $[XXX] |
| Follow-up medication management (20–30 min) | $[XXX] |

Have Questions About Coverage?
We're happy to help you understand your options before you ever come in.
Call (812) 580-9285 →Cancellation & No-Show Policy
We ask for at least 24 hours' notice if you need to cancel or reschedule an appointment. This allows us to offer your time slot to another patient who may be waiting for care.
Appointments cancelled with less than 24 hours' notice, or missed without notice, may be subject to a no-show fee of $[XX]. We understand that emergencies happen — if you have an unavoidable circumstance, please call us at (812) 580-9285 and we'll work with you.
Consistent no-shows affect our ability to serve everyone who needs care. Patients who no-show three times may be discharged from the practice.
To cancel or reschedule, call (812) 580-9285. If you reach voicemail, leave a message with your name and appointment time — cancellations are effective as of the time of your call or message.
Your Right to a Good Faith Estimate
Under the No Surprises Act, if you don't have insurance or don't plan to use your insurance, you have the right to receive a Good Faith Estimate of expected charges before receiving care. This estimate will include expected charges for the services you request.
- You can request a Good Faith Estimate before scheduling a service, or at any time.
- If you receive a bill that is at least $400 more than your Good Faith Estimate, you have the right to dispute the bill.
- Keep a copy of your Good Faith Estimate for your records.
To request a Good Faith Estimate, or to learn more about your rights and the dispute resolution process, call (812) 580-9285 or visit www.cms.gov/nosurprises.
